  • Insurance card(s) and a photo ID (we will make copies)

  • A referral or authorization number, if required by your insurance

  • An up-to-date list of all medications, including vitamins and non-prescription drugs

Providing this information helps us prevent repeat tests and appointment delays.

What heart and vascular conditions do you treat?

Our cardiologists diagnose and treat a full range of heart and vascular conditions, including chest pain, coronary artery disease (CAD), atrial fibrillation (AFib) and other arrhythmias, heart failure, high blood pressure (hypertension), high cholesterol, peripheral artery disease (PAD), leg pain and swelling and varicose veins.

Do I need a referral to see a cardiologist?

In most cases you can schedule directly with us; some insurance plans require a referral, so we recommend checking with your provider. Call (773) 631-0869 and we’ll help.

What insurance do you accept?

We accept Medicare, Blue Cross Blue Shield of Illinois, UnitedHealthcare, Cigna, Humana and Aetna, among others.

Are you accepting new patients?

Yes — we are currently welcoming new patients.

Where are you located?

7447 W. Talcott Ave., Suite 200, in the Norwood Park neighborhood of Chicago, near Edison Park, Park Ridge and O’Hare.

Do you offer telemedicine?

Yes, we offer virtual consultations and follow-ups for patients who can’t come into the office.

What tests can I get in-office?

Stress tests, echocardiograms, vascular ultrasound and cardiac monitoring are all performed in our clinic.
